What is NHTD?

NHTD (Nursing Home Transition and Diversion) is a New York State Medicaid waiver program designed to help individuals who are eligible for nursing home care receive services in their homes or in a community setting.

This program empowers participants to maintain their independence in a familiar and comfortable environment, supported by professional and personalized care services.

Here’s what you required for eligibility:

Medicaid Eligibility

The patient must be a New York resident eligible for Medicaid.

Need for Assistance

Patient must require a level of care that would otherwise necessitate a nursing home stay.

Desire for Community Living

The recovering individual must wish to live in a home or community setting rather than an institution.

Plan Approval

The services must be outlined in an approved plan of care that ensures the individual’s health and safety at home.
